2009 Food Vendor Regulations

Tented spaces will include: 2 tables (1 prep, 1 serving) with cloth and skirting, and two chairs.

Your pricing will be shared with other vendors so that everyone is operating on the same basis.

All vendors MUST remain up and running from 10:00 am until 8:00 pm. Violations may result in a 40% payback to Main Street Medina from your sales.

You must pass inspection by the Medina County Health Department on site before the event begins. The temporary food license will be purchased by Main Street Medina. You must provide all necessary information required for the license application by Monday, August 3^rd, 2009. Temporary Food Service Operation Information Application will be mailed or emailed to participating food vendors.

Be specific in requesting amount of space needed (see attached sheet) if it exceeds the regular booth space.

Requests and type of electrical and water if needed MUST be made in advance. (see contract sheet).
We WILL NOT be able to accommodate requests the day of the event.

You must provide all electrical extension cords (110v and 220v available).

Water is available. You must provide any needed (food-grade) hoses, extensions, etc.

All vehicles must be parked off-site before 9:30am

Volunteers will be available during the day to help remove trash and waste.

Take all used cooking oil and similar products with you after event.

The submission of the application form will be construed as acceptance of all conditions printed on the enclosed information sheet. I understand that neither Main Street Medina, nor anyone affiliated with the Medina Cultural Heritage Day will be responsible for any personal injury, loss, or damage.
